描述一位年長血液透析病人反覆疥瘡感染,造成外籍看護工照護上的困難及壓力,藉由護理措施減輕外籍看護工壓力負荷及避免疥瘡再復發護理經驗。護理期間自2021年7月21日至9月24日,透過身體檢查、觀察、會談及病歷查閱等,運用Gordon十一項健康功能型態評估收集資料,確立健康問題:特定知識缺失、營養不均衡:少於身體需要、照顧者角色緊張。照護期間指導疥瘡相關知識;利用多元指導方式,建立正確飲食習慣達營養需求;並將外籍看護工壓力及需求納入護理計畫,透過不斷的回覆示教,教導個案及外籍看護工學習正確照護,降低主要照顧者因缺乏照護技巧造成心理緊張,進而提升個案生活品質,正向面對透析生活。
This article describes an elderly hemodialysis patient who was repeatedly infected with scabies, which caused difficulties and stress in the care of foreign care workers. Nursing measures were used to reduce the stress load of foreign care workers and avoid the recurrence of scabies. Reducing the stress load of foreign care workers and avoiding the recurrence of scabies through nursing measures. The care period is from July 21, 2021 to September 24, 2021. Through physical examinations, observations, interviews, and medical records, we used Gordon's eleven health function assessments to collect data and identify health problems: deficient knowledge, imbalanced nutrition: less than body requirements, and caregiver role strain. During the nursing period, we teach scabies-related knowledge, and use multiple guidance methods to establish correct eating habits and meet nutritional needs. We incorporate the pressure and needs of foreign care workers into the nursing plan, and teach cases and foreign care workers to learn correct care through continuous feedback and teaching. Through the above methods, the psychological stress caused by the lack of care skills of the main caregivers can be reduced, thereby improving the quality of life of the case and facing the dialysis life positively.
