本文描述關於一位於高溫環境下工作,因熱中暑意識不清,且照護過程中感受到擔憂與不安的病人之處置與照護。照護期間為2021年7月6日14點34分至7月7日14點30分,進行四大層面評估收集資料,確立健康問題有:體溫過高、高危險性損傷、焦慮。透過謹慎執行身體評估,積極降低高溫傷害與補充輸液量以維持腦部、心臟、腎臟足夠組織灌流;建立良好護病關係,提供個人化之熱中暑衛教,提升熱傷害認知及降低焦慮感。地球暖化導致氣溫升高,老年人為熱傷害潛在的高風險群,建議建立熱中暑熱傷害照護課程及醫療處置照護標準,加強護理人員對疾病的認知,於臨床照護時能快速辨識,提供正確診療與評估照護,提升急重症照護品質。
This article describes the treatment and care of a patient who worked in a high-temperature environment, was unconscious due to heatstroke, and felt worried and uneasy during the care. The nursing period is from 14:34 on July 6, 2021 to 14:30 on July 7, 2021. Four levels of assessment and data collection will be conducted to establish health problems: hyperthermia, high-risk injuries, and anxiety. By carefully performing physical assessment, actively reducing high-temperature damage and supplementing the amount of infusion, it maintains brain, heart, and kidney tissue perfusion; establish a good nurse-patient relationship, provide individualized health education about heat stroke, improve awareness for heat damage and reduce anxiety. Global warming leads to rising temperatures, and the elderly are a potential high-risk group for heat injury. It is recommended to establish courses about heatstroke injury care and standards for medical treatment care to strengthen nurses' awareness of diseases, and to quickly identify them in clinical care and provide accurate diagnoses, assessments and treatments to improve the quality of emergent and critical care.
