This case report describes the home care nurse in caring for a motor neuron disease patient with respirator, with regard to hospital discharge planning and following long-term home care. The period of nursing care was from March 24 to December 30, 2001. Observation, interview and physical examination were based on the Neuman Nursing Model to assess nursing problems. Major nursing problems were: impaired physical mobility, deficient knowledge, caregiver role strain, impaired family coping and anticipatory grieving. The home care nurse applied knowledge and nursing skills to assess and provide holistic and continuous nursing care to the patient, and enhance family coping, as well.
