摘要

本篇個案報告是照護一位運動神經元病患的患者，因疾病進展，導致呼吸衰竭長期呼吸器依賴，藉由出院準備服務連結長期照護的居家照護經驗。照護期間為90年3月24日至12月30日，護理過程中依紐曼護理照護模式做整體性的評估，結果發現個案有身體活動功能障礙、知識缺失、照顧者角色緊張、家庭因應能力失能及預期性哀傷等護理問題；護理人員所運用照護重點，除了持續居家照護服務外，更加強家庭照護者的照護技能訓練、善用支持團體及相關的社會資源，協助案家調適面臨疾病所帶來的家庭衝擊及發展家庭照護模式的居家照護經驗。

This case report describes the home care nurse in caring for a motor neuron disease patient with respirator, with regard to hospital discharge planning and following long-term home care. The period of nursing care was from March 24 to December 30, 2001. Observation, interview and physical examination were based on the Neuman Nursing Model to assess nursing problems. Major nursing problems were: impaired physical mobility, deficient knowledge, caregiver role strain, impaired family coping and anticipatory grieving. The home care nurse applied knowledge and nursing skills to assess and provide holistic and continuous nursing care to the patient, and enhance family coping, as well.
