The report described the nursing experience of an end-stage renal disease patient who had kidney transplant after four years' of hemodialysis. Patient's condition was assessed based on Gordon 11-item Healthy Assessment by providing direct clinical nursing care, observations, and interviews. Nursing problems concluded from the assessing data were high-risk infection, defecate pattern altered, anxiety, and knowledge deficit. After receiving proper nursing and health instructions, the patient could face her anxiety, had no complications, and learned how to do self-care. This experience could provide nurses who are taking care of patients with renal transplant to know what problems patients have to face, to make nursing plans properly to promote the patients' quality of life.
