一位行迴腸代用膀胱手術病患之照護經驗

本篇報告是筆者照顧一位罹患膀胱癌並接受迴腸代用膀胱手術患者之護理經驗。在照護期間中（民90.3.8~民90.04.14），筆者運用Gordon十一項功能性健康評估技巧，以觀察、會談及電話訪談方式收集及分析資料，應用護理診斷，確認個案面臨的問題計有：排尿型態改變、低自尊、睡眠型態紊亂等健康問題。在護理過程中，筆者針對個案需求適時地道解其身體症狀、提供資訊及情緒支持，引導個案宣洩其情緒壓力，協助他採取正向因應態度及策略，並協同家屬給予個案不斷之鼓勵與支持，讓病患能在住院期間適應疾病所造成之排尿功能的改變，並成功地恢復自我控制排尿能力。希望藉此個案報告而能與護理同仁分享照護經驗。

In this article, we report the nursing experience in caring for a patient with ileoneobladder replacement. During March 8 to April 14 2001, we applied Gordon 11 item functional assessment instrument, and used observation, bedside interview telephone interview to collect and analyze data. We applied nursing diagnosis to confirm the client's problems to contain: incontinence, low self-esteem, sleep pattern disturbance. In this nursing process, we fitly improved the client's physical symptoms by providing appropriate information and emotional support. Furthermore, we helped client to alleviate stress, to develop positive attitude and coping strategies. After the proper training, the client was able to control his urination with higher degree of satisfaction. During hospitalization, the main frustration happened to the patient was the change of micturition function instead of physical discomfort. Therefore, how to reduce the occurrence of incontience and regain the control of urination is very important. In an attempt to provide better care for the client with urinary diversion, we have discussed in this article.
