摘要

本專案旨在提升「當日入院手術病患」術前準備完整率，為保障病患權益、安全及提高護理服務品質，故引發進行此專案的動機。經由品管小組的查檢發現「當日入院病患」術前準備不完整共81項，完整率為79.42%，分析造成問題的原因，主要包括醫師太晚開立檢驗單、舊病歷未送達、缺乏當日入院手術病患術前準備之工作標準及衛教單。因此專案小組針對上述原因，經討論分析後與醫療科及病歷室溝通協調，共同解決異常的問題並製作當日入院手術病患術前準備之工作標準及手術須知。改進方案實施後，「當日入院手術病患」術前準備完整率，由改善前79.42%提升至改善後100%，進步率為25.91%，目標達成率為100%，確實達到本專案的改善目的。

The aim of this proposal was to raise completion rate of pre-operative preparation for same-day admission patients, as motivated by protecting patients rights, and promoting and ensuring nursing quality. By examining pre-operative preparation for same-day admission patients, there were 81 incomplete items, at 79.42% completion rate. With analysis of the incompleteness, nursing staff found four factors that caused the problem. They were doctors' late laboratory sheets, undelivered old charts and the lack of criteria of pre-operative preparation for same-day admission patients and hygienic leaflets. Therefore, this proposal was to provide suggestions focused on communication and coordination among nursing department, medical departments and medical record room, solving abnormal problems cooperatively, producing criteria and operation notice of pre-operative preparation for same-day admission patients. The completion rate has been raised from 79.42% to 100% after these suggestions were adopted. From the increasing 25.91%, this proposal does achieve its aim to improve pre-operative preparation for patients and raise its completion rate.
