一位肝癌患者手術前後之護理經驗

A Nursing Experience of a Liver Cancer Patient Over the Pre and Post Operative Period

摘要

本個案報告主要目的在探討一位55歲男性肝癌患者手術前、後之護理經驗。筆者採用Gordon十一項健康功能型態之評估架構，於護理期間藉由有系統的觀察、會談及身體評估等方式收集資料，結果發現病人有焦慮、知識缺失、潛在危險性感染、身體活動功能障礙、疼痛、睡眠型態紊亂等護理問題。在為期十天的護理期間，筆者扮演支持、照護及教導者的角色，與個案建立治療性人際關係，照護過程中，考慮病人之個別性需求，讓病人及家屬共同參予照顧計劃，協助個案解決術前之焦慮，且適時給予心裡支持，對於其不了解的問題，提供相關資訊，使其能安心的接受手術，術後在護理問題依序獲得改善及生理功能逐漸恢復後，順利出院返家療養。

The main purpose of this case report is to explore the experience of caring for a 55-yearold male patient with liver cancer over the pre and post operative period. The author used Gordon's eleven health functional types to evaluate and collect information. The results indicate that the patient had nursing problems including anxiety, deficiency of knowledge, potentially hazardous bleeding, potentially hazardous infection, impaired physical mobility, pain, sleep disturbance, etc. Due to the therapeutic relationship with the patient that was established during the period of providing nursing care, the individual requirements of the patient were specifically considered and assistance in relieving the anxiety of the patient before the operation was provided. Furthermore, mental support was provided opportunely and relevant information offered so that the patient was assured before the operation took place. After the operation, the nursing problems were acted upon as they occurred in order to improve the patient's wellbeing and the physiological functions of the patient were restored gradually.
