一位脊髓損傷併發褥瘡患者之護理經驗

Nursing a Patient with Spinal Cord Injury and Decubitus Ulcer

摘要


脊髓損傷患者常因為臥床或肢體癱瘓致褥瘡的產生。本報告是探討一位49歲脊髓損傷之女性患者,於照顧期間以Gordon十一項健康功能評估架構,確立病患健康問題為:組織完整性受損、身體心像紊亂、無力感等健康問題,在護理過程中運用主動關懷、同理及接受等技巧與個案建立良好護病關係,除提供個案生理照顧外,並扮演支持者、傾聽者的角色,引導個案說出害怕及擔心的事,並鼓勵正視身體的改變,運用支持系統提供情緒支持,協助個案克服疾病所帶來之衝擊,使其能更積極接受治療,進而提昇其生活品質。

Spinal cord injuries often lead to decubitus ulcers due to paralysis or lying in bed for extended periods. This essay discusses the case of a 49-year-old female with SCI who was assessed according to Gordon's framework of Eleven Health Functions during her treatment. The health problems confronting this patient included impaired tissue integrity, body image disturbance, and powerlessness. With solicitude, empathy and acceptance, therapists are better able to establish good nurse-patient interactions to facilitate a nursing program. In addition to providing physical care, the nursing staff ought to play the role of supporters and listeners, guiding patients with SCI to confess their fears and worries. Moreover, it is very important to aid individuals with SCI to understand and confront the changes in their bodies. Delivering emotional support and assistance for overcoming the impact of disease is necessary to encourage people with SCI to undergo therapy and improve their quality of life.
