台灣醫療機構護理主管對專科護理師之認知

摘要

專科護理師制度的推動是衛生署近年來重要政策之一，主要爲提高護理人員專業素養，與提升民眾的照護品質。本研究以焦點團體法了解台灣醫療機構護理主管對專科護理師的認知。分別在北、中、南三區邀請22位不同層級醫院之護理主管，參與焦點團體訪談，其中醫學中心7人，區域醫院7人，地區醫院8人。將訪談內容轉錄爲行爲過程紀錄，所得資料以內容分析法分析，結果歸納爲二大類：台灣專科護理師現況與對台灣專科護理師未來的期望。前者含五個主題：醫師不足，護理代職；專業發展，人員成長；各取名稱，爲找出路；管理不易，心歸何處；及工作職責，天壤之別。後者含二個主題：既期待又怕受傷害；與有法律保障，走得更長遠。本研究結果可提供醫院護理主管規劃專科護理師職責及衛生署立法的參考。

The use of nursing practitioners has been one of the important policies initiated by the Department of Health Bureau in recent years. This approach should improve nursing professional competence and promote quality of care. The purpose of this study was to understand the perceptions held by hospital nursing managers about nurse practitioners; this was carried out using focus groups. In total, 22 hospital nursing managers from the northern, central, and southern areas of Taiwan were invited to take part in the study. Seven of the nursing managers came from medical center hospitals, seven came from area hospitals, and eight came from local area hospitals. The interview contents were collected verbatim and then analyzed. The results fell into two categories; the first was the managers' opinions of current situation with respect to nurse practitioners in Taiwan and the second was the managers’ future expectations of nurse practitioners in Taiwan. The former category could be divided into five themes: doctor shortage and nurse substitute, professional and personal development, variable names for the same job, managerial problems caused by a loss of identity, and the designation of responsibility. The latter category had two themes: expectation versus injury and the hope of protection from legal problems. The results of this research will provide a reference when establishing the job description of and setting the regulations for nursing practitioners.
