提升大腸直腸外科病房手術病人術前準備完整性之改善專案
Enhancing the Rate of Complete Preoperative Preparation for Patients in the Colorectal Surgical Unit
摘要
完整的術前準備，對病人手術過程的安全及手術是否順利進行，有著密切關係。本專案旨在提升大腸直腸外科病房手術病人術前準備之完整性，經現況分析發現2008~2009年期間，術前準備不完整率爲0.47%(23/4,851)，其原因包括：交班不完整、確認資料不完整、檢查病人身上飾品不完整、病人不了解需配合事項。因此本專案運用品質改善之專案手法，制定術前準備流程、修訂術前衛教資料，透過手術病歷資料及同意書核對工具、設計術前準備提醒標語、創意設計術前補給站、提供人形圖卡之勾選及核對，以及主動邀請病人參與術前準備過程，不僅有效減少術前準備異常，落實病人手術安全之目標，也提升了護理人員之護理品質與工作效率，臨床術前準備工作帶來莫大的助益。

A complete pre-operative preparation is directly related to the intraoperative safety and progression of patients. This project was aimed at enhancing the completion rate in pre-operative preparation for patients in the colorectal surgical units. Current analysis had revealed that the rate of incomplete pre-operative preparations was 0.47% (23/4,851) between 2008 and 2009. The causes were: incomplete shift transfer procedures, incomplete data confirmation, incomplete removal of personal items from patients, and poorly informed cooperating procedures. The project had applied Interventions including: drafting pre-operative preparation procedures, revising pre-operative health education material, developing checklists based on pre-operative medical records and consent forms, designing pre-operative preparation reminder slogans, designing innovated pre-operational information counter, providing checking cards with human figure, and inviting patients to participate in pre-operational preparations. This project effectively reduced incompletion rate in preoperative preparation and ensuring patient safety in surgical procedures. Moreover, the results of this project indicated an improvement in nursing care quality, working efficiency, and brought about tremendous benefit to pre-operative preparation.
