母乳是早產兒重要的營養來源,本單位2009年早產兒純母乳哺育率只有23.3%;因此,專案主旨為提升新生兒加護病房早產兒純母乳哺育率。現況發現護理人員對產婦及家屬哺餵母乳護理指導的執行率僅58.1%、護理人員對早產兒純母乳哺育之認知正確率63.5%,追蹤產婦及家屬執行母乳哺育率僅29.5%。專案內容包括舉辦制定早產兒純母乳哺育衛教流程、製作早產兒純母乳哺育衛教範本、舉辦早產兒純母乳哺育衛教課程、定期稽核、衛教及袋鼠護理、未前來會客的產婦及家屬及時給予電話訪查等。改善措施後,護理人員純母乳哺育的執行率由58.1%提升93.1%,早產兒純母乳哺育率由23.3%提升67.5%。照護人員可應用專案內容於臨床工作中,以改善早產兒純母乳的哺育率。
Breast milk is an important nutrition source to premature infants, but the exclusive breastfeeding rate was only 23.3% at our neonatal intensive care unit (NICU) in 2009. The aim of this project was to increase the breastfeeding rate for premature infants in NICU. Before this project was implemented, only 58.1% of mothers and their families were given breastfeeding instructions for the premature infants, nurses had a correct response rate of 63.5% in knowledge about breastfeeding premature infants, and only 29.5% of mothers and their families were followed for their breastfeeding practices. During this project proceeding, we had several improvements, include organizing lectures where nurses conduct breastfeeding education for mothers of premature infant, drafting breastfeeding checklist, conducting phone interviews with postnatal mothers at home, drafting guidelines on breastfeeding premature infant, and encouraging family members to participate in breastfeeding classes and kangaroo care. After these improvements were implemented, the execution rate of breastfeeding instructions to mothers and their families by the nursing staffs improved from 58.1% to 93.1% and the breastfeeding rate for premature infants increased from 23.3% to 67.5%. Results from this program can be applied to clinical settings to improve the rate of breastfeeding by mothers to premature infants.
