急診外傷病人護理指導之改善專案

Improving the Nursing Instruction of Emergency Trauma Patients

摘要


本專案旨在提升急診外傷病人出院品質,因外傷病人對傷口照護有疑惑及出院後常接到電話諮詢傷口處理及注意事宜機率高,故引發改善動機。經現況分析後發現,急診外傷病人對於照護認知不足、缺乏完善的護理指導規範及審核機制、執行出院計畫不完整且缺乏對出院計畫評估之認知,以致於執行護理指導率偏低。提出解決辦法如下:(1)規劃外傷在職教育訓練;(2)制定外傷病人護理指導考核機制;(3)建立標準化外傷病人護理指導標準;(4)規劃進行護理指導地點;(5)訂定傷口照護病人後續追蹤機制等,措施進行半年後,外傷病人傷口指導完整率從改善前的22%提升至90%,達成專案目的,不僅提升照護品質並且提升病人滿意度。

This project was conducted to improve the quality of hospital discharge for emergency trauma patients. Trauma patients often have doubts about wound care, and the rate of enquiring about the method and instructions for wound care via phone after leaving the hospital is high; this triggered the motivation for improvement. After analysis, we found that the trauma patients did not have enough knowledge on wound care and that the standard of nursing care guidance was lacking. The examining, verifying and executive mechanisms were incomplete; furthermore, evaluation of the discharge plan was not performed, resulting in a low instruction rate for nursing care. Therefore, we proposed the following solutions: (1) Standardize in-service education about trauma nursing. (2) Establish an evaluation mechanism for trauma nursing instruction. (3) Establish a standardized trauma nursing guidelines. (4) Plan a venue for performing nursing training. (5) Establish a follow-up mechanism for patients who require wound care. After implementing these solutions for half a year, the completion rate of wound care instruction for trauma patients increased from 22% before improvement to 90%, achieving the aim of this project. We observed improvements in the quality of care and increases in the degree of patient satisfaction.
