一位腦中風患者及其主要照顧者之照護經驗

A Nursing Experience of a Stroke Patient and the Primary Caregiver

摘要


本文為協助一位腦中風病患及主要照顧者增進疾病及復健照護認知,以預期目標為導向,使個案能執行適當的自我照護行為之護理經驗。護理期間自2011年3月21日至4月30日,應用Orem自我照顧理論進行整體評估,發現個案有自我照顧能力缺失、照顧者角色緊張、知識缺失的問題。筆者以個案管理方式,整合醫療資源,與個案及家屬共擬治療及復健方針,協助調適因疾病導致之活動障礙,引導配合復健增加自我照顧能力;教導相關照顧技巧,緩解照顧者角色緊張與焦慮;採漸進及重點式衛教,累積對疾病與復健的認知。提供社會福利資源與出院準備服務,透過電訪追蹤,給予適切後續照顧,讓家庭回歸正常運作。

This article described the nursing experiences of assisting a stroke patient and his caregiver to acquire self-care ability by enhancing their recognitions of cerebrovascular diseases and the related rehabilitation therapy with expectation-based method. From March 21th to April 30th 2011, the author applied Orem's self-care theory to identify the primary problems of this case, which included self-care deficits, caregiver role strain, and insufficient care knowledge. By implementing the case management methods, the author integrated all available medical resources and discussed with the patient and his family on collaborative treatment and rehabilitation plans. Rehabilitation programs were applied to help the patient to improve motor functions and enhance self-care ability. In addition, the caregiver was educated on care techniques to ease caregiver role strain and anxiety, as well as step-by-step nursing education for building up the patient and caregiver's knowledge of the disease. Finally, the author offered social welfare resources, conducted discharge planning, and applied telephone interviews to provide appropriate follow-up care and return the patient and his family to normal life.
