提升護理人員執行腦中風病人吞嚥訓練完整性
A Project for Improving the Completeness of the Swallow Training for Stroke Patients among Nurses
摘要
吞嚥困難為腦中風病人常見症狀，會導致肺炎、脫水與營養不良等，及早提供吞嚥訓練，有助於吞嚥功能恢復，進而增進生活品質。本專案旨提升護理人員協助腦中風病人執行吞嚥訓練完整性，現況發現腦中風病人吞嚥訓練完整性僅達54%，原因為吞嚥訓練認知不足，缺乏復健師與護理師間交接吞嚥治療情形、缺乏吞嚥訓練相關衛教單張及稽核機制。擬定對策為舉辦「教育訓練」、「建置吞嚥訓練護理資訊作業系統」、「製作多媒體教學光碟」、「製作吞嚥訓練衛教指導手冊」及制定「吞嚥訓練完整性稽查作業」。經專案實施後，整體完整性由54%提升為100%，達成專案目的。使病人得到安全照護，並運用資訊化數位管理促使醫護間有效溝通，進而提升照護品質。

Dysphasia is a common symptom in stroke patients, and is known to cause pneumonia, dehydration and malnutrition. The early swallowing training will improve the recovery of swallowing function, and thus enhance the quality of life. The project aimed to enhance the nurse's ability to help stroke patients to perform the complete swallow training. Currently, the completeness of swallow training for stroke patient is as low as 54%, primarily due to nurses' lack of knowledge in swallow training, inefficient handover between physical therapist and nurses on patient's swallowing condition, and lack of health education pamphlets and auditing mechanisms. After analyzing the current procedures, the proposed improvement approaches included: arranging continue education training for staffs, developing swallowing training information guideline, and constructing instruction checklist. After the implementation of the project, the completeness of the swallowing training for stroke patient increased from 54% to 100%, which achieved the project's intended purpose. The patients now receives safe care, and the health care team could now communicate effectively and provide higher quality medical care by implementing digital information management.
