A Nursing Experience of a Spinal Cord Injury Patient due to Car Accident

摘要

本文主要描述一位因車禍致脊髓損傷患者之照護經驗。個案因突然的車禍致四肢癱瘓造成身心問題，短期之內需家人全責照護，但家屬初次照護行動不便的病人，因而遭遇照護困難及心理衝擊。照護期間自2011年8月19日至2011年9月14日。筆者運用關懷及同理與個案及家屬建立治療性人際關係；運用身體檢查、Gordon十一項功能性健康型態評估等方式收集資料，確立個案有身體活動功能障礙、無力感、照顧者角色緊張等健康問題。在照護期間協助進行復健活動，以提升個案現存功能與減輕殘障；引導個案及家屬表達內心感受，除幫助建立信心與控制感，亦協助家屬學習如何照顧個案，使其能以正向積極的態度面對未來的療程及復健。本文能提供護理人員在照顧類似個案時之參考。

This case report aimed to describe the nursing experience of a quadriplegic patient resulted from spinal cord injury induced by a car accident. The patient, suffering from psychological and physiological issues post-accident, required immediate and total care from his family, yet the family was encountering nursing difficulties and psychological impacts due to inexperience. The nursing period was from August 19, 2011 to September 14, 2011. The author developed a positive nurse-patient rapport based on caring and empathy. Data were collected via physical examination and Gordon's functional health pattern assessment framework, and the following health issues were confirmed: impaired physical mobility, powerlessness and caregiver role strain. During nursing intervention, the author assisted the patient and his family with rehabilitation exercise to promote physical functions and to mitigate disability, guide them to express their feelings in order to gain confidence and sense of control, and aiding the family in deriving a individual care plan, so the patient and the family can cope with future treatment and rehabilitation with positive attitude. This case report could serve as a reference for nursing staff when attending similar cases.
