提升下肢骨骼牽引照護正確率之改善專案

摘要

下肢骨折病人常執行骨骼牽引緩解疼痛及促進骨折復位，牽引效能及照護不佳將導致合併症的發生，所以正確的骨骼牽引照護相當重要。臨床照護發現骨骼牽引病人脛骨處常發生壓瘡及牽引功能不佳，故成立專案小組，目的希望提升牽引照護的正確性。經現況分析發現護理人員對骨骼牽引照護認知率(65.31%)及照護的正確率均低(71.7%)。經改善措施：舉辦在職教育課程、骨骼牽引技術執行方法及異常處理參閱本的應用及牽引照護臨床實務評核表，使護理人員牽引照護認知顯著提升至95.87%、骨骼牽引照護正確率提升至96.7%達成專案目的。此專案有助提升護理人員牽引照護認知及執行照護措施的正確性，改善病人牽引處置之舒適及功能，增進照護品質。

Skeleton traction is often used in lower limb bone fracture to relieve pain and improve fracture reduction. Inappropriate traction care may cause substantial morbidity, such as pressure sores and the delay of rehabilitation. Therefore, we established a project team to enhance the accuracy of the traction care. The analysis of current situation revealed that the perception rate and the accuracy rate of skeletal traction care were 65.31% and 71.7%, respectively. The Improvement measures include: organizing in-service education programs, providing reference manuals of bone traction skills and fracture management and checklists of traction care. The perception rate and the accuracy rate of traction care nurses has been significantly improved to be 95.87% and 96.7%, respectively. This project could be helpful to improve traction care, the accuracy of care procedure, the comfort and function of the traction patient.
