一位青少年因意外導致右膝下截肢之護理經驗

本文是描述一位青少年因車禍導致右膝下截肢，截肢後面對活動受限及身體心像改變故個案有哭泣及退縮行為，促使筆者想協助適應生活的改變及重建正向的身體心像，筆者於2013年9月21至10月7日護理期間，以直接照護評估、會談、病歷查閱及與家屬互動方式，運用Gordon十一項功能性健康型態進行評估，發現個案有疼痛、身體活動功能障礙、身體心像紊亂之健康問題。照護過程中予主動關懷建立良好護病關係，運用疼痛控制技巧緩解個案術後疼痛不適；適時復健以維持四肢肌力和關節活動度並提供輔具相關資訊，讓個案盡早恢復以往獨立自我照護生活；鼓勵個案及家屬適度的情緒宣洩，製作相同境遇案例剪報及社會資源，用正向態度面對因患病過程導致生活巨變的事實，重建自我角色功能與價值感，提高日後生活品質及良好社會適應。

The report described the nursing experience of a teenager who, after undergoing a right below-knee amputation as a result of a car accident, began to exhibit crying and social withdrawal due to limited mobility and body image disturbance. From Sep. 21^(st) to Oct. 7^(th), 2013, the author collected information through direct care, interview, medical records, and family interactions, and applied Gordon's 11 functional health patterns to assess the patient's conditions. The following health problems were confirmed: pain, physical activity dysfunction, and body image disorder. During the nursing period, the author offered active care-giving and established a positive nurse-patient relationship, reduce post operative pain and discomfort through pain control techniques, facilitate self-care and independence by discussing appropriate rehabilitation therapy with the patient and therapist and providing information of assistive devices, relieve the patient and family stress with appropriate emotional release, encourage positive thinking and rebuilding by compiling newspaper clippings of patients with similar experiences and available social resources, and rebuild autonomous functioning and sense of value to enhance life quality and improve social adaptation. This report can serve as a reference for future nursing practice.
