運用母嬰親善協助一位產婦成功哺餵母乳之護理經驗

A Nursing Experience of Using Baby-Friendly Skills to Successfully Assist a Mother in Initiating Breastfeeding

摘要


本篇敘述一位43歲高齡產婦,相隔20多年再度生產,第一次學習哺餵母乳,於產前門診衛教時,一直表示強烈的哺乳意願,卻總是缺乏自信而感焦慮,擔心不會成功、擔憂奶水不足,產後又因為寶寶喝不到奶水,求助於護理人員的護理經驗。筆者接觸個案於2013年7月23日產檢時,故護理期間為2013年7月23日至2013年8月14日,應用Gordon十一項功能健康評估發現個案有1.焦慮;2.低效性母乳哺餵;3.疲憊之護理問題,因此運用母嬰親善技巧,與個案分享哺餵母乳之經驗,並藉由產前門診及產後結合母嬰親善團隊予護理指導,提供哺乳與育嬰的相關知識及技巧,使個案對哺餵母乳獲得信心進而感到滿足有成就感,至今仍持續哺餵母乳中,並與同仁分享育嬰和哺乳的喜悅。

This article reported a 43-year-old mother seeking help from nurses for breastfeeding difficulty. 20 years after her prior birth, the mother, expressing strong will to breastfeed at the prenatal clinic, learned about breastfeeding for the first time. However, she experienced constant anxiety and self-doubt, and was concerned with the risk of breastfeeding failure and insufficient lactation. After the delivery, she sought medical assistance because the infant was not receiving enough breast milk. The author approached the mother during the prenatal examination on July 23, 2013, and from July 23 to August 14, 2013, collected data with Gordon's 11-item functional health patterns. The patient was found to have: 1. anxiety; 2. ineffective breastfeeding; and 3. fatigue. Through the application of baby friendly skills and sharing breastfeeding experience, as well as providing nursing instructions on breastfeeding and infant care during prenatal clinic and by the postpartum baby-friendly team, the mother acquired confidence in breastfeeding and felt satisfied from her accomplishments. She continues to breastfeed her infant and to share her experience and joy with our nursing staff.
