抽痰對早產兒來說是一種侵入性治療。不正確的抽痰方式可能會引起早產兒呼吸暫停、心律不整、肺塌陷,甚至死亡。專案實施前,新生兒加護病房護理人員執行早產兒抽痰照護正確率僅65%;為改善新生兒加護病房護理人員執行早產兒抽痰照護正確率,我們設計了本專案。經由實際操作訪查以及試卷考試的方式評估,並針對各項缺失提出解決方案,例如舉辦在職教育訓練、制定早產兒抽痰照護指引、增修抽痰術標準作業規範、製作抽痰深度提示卡和抽痰管測量卡、設置抽吸壓力錶裝置及標示。專案實施後,新生兒加護病房護理人員執行早產兒抽痰照護以及知識的正確率分別由65.0%至94.5%,以及 66.3%至96.6%。我們期盼藉由此專案,增進護理人員的專業知識與技能,以及早產兒的照護品質。
Sputum suction is considered as a form of invasive treatment for preterm infants, where incorrect execution could lead to apnea, arrhythmia, lung collapse, and even death. The project was designed and implemented to improve the success rate of neonatal intensive care nurses in sputum suction of preterm infants, where, prior to the project, was only 65.0%. Assessment was conducted through on-site investigation to questionnaires and each identified flaw was addressed with solutions like the introduction of in-service program, nursing guide to sputum suction on preterm infants, standard practice of sputum suction, sputum suction cue cards and measurement cards, and installation and labelling of suction pressure gauge. The project, after its implementation, had increased the success rate and cognition of neonatal intensive care nurses in sputum suction in preterm infants from 65.0% and 66.3% to 94.5% and 96.6% respectively. We hope to improve the professional knowledge and skills of nurses and the nursing quality of preterm infants by sharing this project.
