A Post-Anesthesia Nursing Experience of a Preschooler Suffering from Burn Undergoing Debridement

摘要


本文是探討一位學齡前期兒童燙傷清創手術後之恢復室照護經驗。筆者於2015-09-25 12:50~13:50共照護一小時,評估個案有疼痛、焦慮、照顧者角色緊張等問題。筆者以疼痛臉譜量表有效評估個案疼痛,及有趣的冰原歷險記影片、音樂-成功轉移個案對疼痛之注意力;以治療性遊戲-有效緩解個案對住院打針及陌生環境的焦慮;鼓勵案母說出內心感受、指導並提供燙傷傷口術後照顧相關資訊,降低照顧者緊張。在衛教影片部分受限於院內觀看,若能以QR cord方式讓案母掃描後於院外隨時操作觀看,有助於提升照顧者對疾病知識、醫療照護訊息的獲得,降低照顧者的壓力、角色功能盡早適應,此部分會提出與單位主管討論,建議院方採行。

This article described a post-anesthesia nursing experience of a preschooler suffering from burn undergoing debridement. The nursing period was from 12:50-13:50, September 25^(th), 2015. The health problems were identified as followed: pain, anxiety, stress, and caregiver role strain. The author effectively assessed the patient's pain level using face rating scale, and distracted the patient from pain by playing the movie "Ice Age" and the soundtrack. The anxiety toward being hospitalized and unfamiliar environment was also eased by therapeutic play. In order to relieve the caregiver role strain, the author encouraged the family to express their feelings, provided them with information and instruction on postoperative wound care, and involved the family in the care process. A problem the author had noticed in the nursing process was that the health education video can only be viewed inside the hospital. The problem could be solved if the caregiver could access the video by scanning a QR code, as it would enhance the cognition of the caregiver on the disease and relevant medical information that would reduce caregiver role strain and facilitate role adaptation. The author will discuss the request with the department supervisor and recommend the hospital to adopt the new policy.
