運用性別友善概念照護一位反覆感染性病患者之經驗

A Nursing Experience of Providing LGBT-Friendly Care for a Patient with Recurrent Sexually Transmitted Infection

摘要


本篇主要描述一位從事不安全的性行為之同志個案,在反覆感染肛門濕疣之護理經驗,自2016年1月21日至2016年4月5日以直接護理、病歷回顧、觀察、會談、電訪、訪視等方式進行資料收集,運用身體、情緒、智能、社會及靈性等五大層面進行評估,確認主要健康問題有高危險性活動、焦慮及無望感等。於照護過程中,除提供個案適當的護理處置外,適時澄清其疑慮、提供跨領域醫療團隊服務,讓個案重新認識高危險性的性活動與反覆感染性病之相關性,教導正確且安全的性知識與同志性活動,透過團體座談活動了解愛滋感染與因應的策略,來協助個案走出無望感,在照顧期間確保隱私權,提供零恐懼等相關護理措施以降低其焦慮。

This paper described a nursing experience of a homosexual patient with recurrent condyloma acuminata who had been engaging in unprotected sexual activities. The nursing period lasted from January 21^(st)to April 5^(th), 2016. Data were collected via direct care, medical record review, observation, interview, phone interview, and home visit, and were assessed in five major dimensions: Physically, emotionally, intellectually, socially, and spiritually. The main health issues were identified as high-risk sexual behavior, anxiety, and hopelessness. During the nursing process, the patient was provided with appropriate nursing measures, timely clarification of any inquiries, and interdisciplinary medical services, which prompted the patient to recognize the correlation between high-risk sexual behaviors and the recurrence of sexually transmitted infection. The patient was then taught safe-sex knowledge and homosexual sexual activity, as well as AIDS infection and its coping strategies through group discussions, as a way to assist him to counter hopelessness. The patient's privacy was protected throughout the nursing period, and non-discriminatory measures were taken to reduce the patient's anxiety.
