提升護理人員執行急重症病人轉入加護病房交接照護完整性

正確且完整的交接是病人安全照護中重要的一環，交接作業不完整，易導致異常事件發生而影響病人安全。調查發現急重症病人轉入加護病房交接照護完整性僅達74.7%，經現況分析問題為轉入交接照護認知不一致、需交接項目繁多及缺乏組織分工。本專案旨在提升急重症病人轉入加護病房交接照護的完整性，以確保病人安全及提供最佳的醫療護理。2015/05/01起經舉辦轉入病人交接照護醫護討論會、制訂轉入病人交接照護指引單、修訂轉入加護病房病人交接照護流程圖及設計工作分配口訣等改善方案，轉入護理交接照護完整性由74.7%提高至97.5%，經由專案改善可有效提升轉入病人交接照護的完整性，促進病人安全並可作為其他加護單位參考，共同提升照護品質。

Accuracy and completeness during transition play a critical role in patient safety. Incomplete transition may lead to the occurrence of abnormal events that may compromise patient safety. Upon investigation, the completion rate of critical patients transfer from a ward into ICU was merely 74.7%. According to further analysis, the issues were identified as inconsistency during ICU to ward transfer, excessive items during transfer, and absence of assignment of responsibility. The purpose of this project was to improve the completion rate of transition nursing from wards to intensive care unit, and therefore secure patient safety and quality care. Starting on May 1^(st), 2015, a seminar on ward to ICU transfer was held, a guideline for the transfer was established, a flow chart was designed, and work distribution mnemonic phrases were introduced. The transfer completion rate was improved from 74.2% to 97.5% after the intervention. The project had significantly enhanced the transfer completion rate and facilitated patient safety during transfer. We hope this project can be a reference for ICU nurses in an collaborative effort to improve the quality of transition nursing.
