運用自立支援照顧模式於年輕型腦中風之護理經驗

摘要

本文描述一位年輕型腦中風之照護經驗。照顧期間於2018年04月06日至2018年04月27日止，筆者藉由直接照護、會談、觀察及家屬訪談、病歷查閱等收集資料，確立有身體活動功能障礙、自我照顧能力缺失、無力感等護理問題。筆者運用自立支援照顧模式照護，提供個別性護理措施與心理支持，並依患者認為「最重要的事」做為「生活功能自立訓練」之目標，增強患者接受正、負面情緒表達。護理措施包括：護理指導和護病互動，鼓勵主動參與日常自我照顧，正向面對疾病衝擊，保持對未來的積極態度與陪伴患者克服身心不適。最終達成所設定生活功能自立之目標，因此，分享此成功經驗以供相關醫療團隊參考。

The purpose of this paper is to describe the nursing experience of using a self-supporting care model to care for a 43-year-old patient with cerebrovascular accident from April 6 to April 27, 2018. We collected the patient's data via direct nursing care, observations, medical records, and interviews from the patient and wife. The main health problems of the patient were physical dysfunction, self-care deficit, and powerlessness. The self-supporting care model used by the author was based on ＂the most important things＂ to the patient. To establish trust and a therapeutic relationship between patient and nurses, we aimed to enhance the patient's expression of positive and negative perceptions. The individual nursing intervention included: proper nursing instructions and facilitation of a healthy nurse-patient relationship to encourage the patient to perform daily self-care, to face the impact of physical dysfunction, and to keep a positive attitude in the future and prognosis of stroke. We also accompanied the patient throughout the process to overcome the physical and psychological discomfort. Hence, this study can provide a reference for nurses taking care of a young patient with stroke.
