Perioperative Nursing Care of a Patient Who Underwent Arthroscopic Shoulder Labral Surgery

摘要

本文描述一位因運動傷害導致肩關節盂唇撕裂病人之手術全期護理。於2017年11月2至11月5日病人住院期間，透過術前身體評估、術中整合性評估和術後訪視，歸納出個案有三個主要護理問題：（1）焦慮（2）潛在危險性傷害（3）急性疼痛。針對這些問題，提出照護方法：（1）術前提供關節鏡相關資訊和指導，來減輕其焦慮感；（2）術中提高關節鏡沖洗液溫度藉此維持個案體溫及減少併發症產；（3）術後量化肩部腫脹情況，並藉藥物及冰敷以緩解疼痛。當病患回到病房後，與醫師和物理治療師共同為個案擬定合適的復健計畫，期讓個案盡早重返運動場。希望藉此經驗提供手術全期的照護內容，提升病人安全，做為日後照護相關病患之護理人員之參考。

This study describes the perioperative nursing care of a patient who underwent arthroscopic shoulder labral surgery after a sports injury. The patient was hospitalized from 2017/11/2 to 2017/11/5. The patient's information was obtained through preoperative physical examination, intraoperative vital signs monitoring, and postoperative visits. The main health issues identified were anxiety, risk of injury, and acute pain. The interventions included (1) providing detailed information and instructions about the arthroscopic operation to reduce the patient's anxiety, (2) warming up the arthroscopic irrigation fluid to maintain the patient's body temperature and avoid complications during the operation, and (3) assessing the patient's shoulder for swelling and pain. After the patient was sent to a general ward, the nurse discussed with the physician and physiotherapist to figure out a suitable rehabilitation plan to help the patient recover more quickly and play sports soon. This nursing experience serves as a guide for nurses to develop effective full-term care for similar patients in the future.
