本文描述一位男性愛滋病合併卡波西氏肉瘤傷口感染之急診照護經驗,照護期間自2019年03月16日09:10至03月18日15:00,經直接護理、觀察、會談、身體評估、病歷查閱及後續追蹤等方式收集資料,運用Gordon十一項健康功能型態評估,發現個案有急性疼痛、現存性感染、無望感等健康問題。筆者在急診照護過程感受個案因傷口感染及疼痛不適為立即性問題,介入傷口護理,落實感染控制原則,避免個案二次感染及教導減輕疼痛技巧。在護理期間與個案建立良好護病關係,鼓勵表達自我感受,建立家庭支持系統,與愛滋病個管師共同討論治療計畫,降低個案對疾病無望感及增進自我信心,讓個案早日回歸正常生活,也期望此護理經驗提供急診護理人員日後照顧之參考。
This article described the emergency care experience of a patient with AIDS combined with Kaposi's sarcoma wound infection. The nursing period lasted from 09:10 on March 16, to 15:00 on March 18, 2018. Data were collected by direct nursing care, observation, interviews, and physical assessment. The author utilized medical records and Gordon's 11 functional health patterns for a comprehensive assessment which confirmed high priority problems including acute pain discomfort, existing wound infections, and hopelessness. For wound infection we implemented infection control principles for changing wound dressing to avoid secondary infections and taught the patient pain relief techniques. During the nursing period we establish a good relationship with the patient, and the author encouraged the patient to express his feeling and established his family support system. To reduce the patient's hopeless of disease and enhance patient's self-confidence, the health professional team discussed the treatment planning with the AIDS case manager. Ultimately, the patient was successfully discharge and recovered gradually. This nursing experience may be a helpful clinical reference for nurses to provide emergency care in the future.
