印尼籍白血病患者來臺接受異體造血幹細胞移植之護理經驗

Nursing Experience of Indonesian Leukemia Patient Receiving Allogeneic Hematopoietic Stem Cell Transplantation in Taiwan

摘要

本文是描述患有急性淋巴性白血病男性，來臺接受異體造血幹細胞移植的護理經驗。照護期間自2020年8月31日至9月28日，運用羅氏適應模式進行整體性評估，確認健康問題：（一）口腔黏膜破損（二）疲憊（三）焦慮。筆者與醫療團隊討論設計提問單增加醫病溝通，進行跨團隊照護，依適切性介入營養師、心理諮商及中醫師，減緩焦慮；並提供造血幹細胞移植照護手冊以提升自我照顧能力。建議臨床繼續教育應包含心理相關課程，如正念訓練技巧，可引導個案表達其焦慮並與醫護團隊討論介入措施，以減緩不安、負向情緒表現；期望此篇經驗可做臨床照護之參考。

This case report describes the nursing experience with a male patient with acute lymphoblastic leukemia who underwent allogeneic hematopoietic stem cell transplantation in cross-national. The nursing period, from August 31 to September 28, 2020. Roy's adaptation model was used as an evaluation tool in a comprehensive assessment of health issues which included, (1) oral mucosal injury, (2) fatigue, and (3) anxiety. The author discussed with the medical team to design a questionnaire to increase communication between doctors and the patient, to provide cross-team care, and to intervene with dieticians, a psychological counselor and Chinese medicine practitioner, according to suitability to reduce anxiety. And to provided hematopoietic stem cell transplantation nursing manuals. These measures improved the patient's ability for self-care. It is recommended that continuing education should include psychologically relevant courses to help patients in expressing anxiety and discussing interventions with the health care team to reduce anxiety and depression. This experience is shared in the hope of extending references for clinical care.
