提升COVID-19疫苗接種作業執行正確性之專案

在COVID-19疫情嚴峻且醫院人力吃緊下，同時為了提高疫苗覆蓋率，配合政府緊急於院外設立疫苗接種站，但一個月內即接到健康中心通知有9例不符合現行政府規定之疫苗種類混打的給藥異常案例。為提升COVID-19疫苗接種作業執行正確性，分析主要原因有：所有疫苗都在同一櫃台報到、標示及動線分流不清楚、無法即時查詢全國疫苗注射系統、每日輪替支援人力、不清楚現行疫苗接種政策、未提前接受疫苗接種作業教育訓練、不清楚現場工作分配。經由專案介入措施：調整作業流程改善報到環境及動線、全面啟動預防接種紀錄即時查詢系統、工作人員在職教育訓練。專案實施後COVID-19疫苗接種作業執行正確率由78%提升至98.6%達到專案目的，值得推廣應用並做為其他醫療單位之參考。

During the impact of the severe transmission of COVID-19, the government was committed to increase the vaccination coverage. Because of the healthcare staff shortage, the government urged the set up of out-of-hospital vaccination stations. However, the healthcare centre identified 9 abnormal Mixing and Matching cases within one month. In order to promote the accuracy rate of vaccination, this research identified main factors as: all vaccine takers were guided to check in at one counter; the guidance signs and routes were not clear enough; there was no access to vaccine verification system; the daily rotation of supporters; lack of knowledge of current vaccine policy; no pre-education or training for medical workers; and limited knowledge of on-site cooperation and division. Actions taken to improve accuracy rate included: adjusting the process, improving the environment and the routes of vaccination stations; launch the on-line vaccine verification system; and enhance the pre-education or training for medical workers. After above adjustments were conducted, the accuracy rate had increased from 78% to 98.6%. This research provides the valuable reference for other medical institutes.
