協助一位腦瘤合併失語症個案調適之護理經驗

摘要

本文旨在描述一位羅患腦瘤合併失語症個案，在住院期間的調適過程。本個案為23歲年輕女性，因腫瘤壓迫導致失語症，筆者於2004年5月17日至2004年6月16日照護期間，於某醫院神經內科病房依據羅氏適應模式，藉由身體評估、會談、觀察等方式進行資料收集，得知個案因身體心像的改變、角色任務失敗造成無望感等護理問題。護理重點先經由完整評估及提供措施，幫助家屬了解個案的身體症狀及功能損傷程度，提供家屬知識及照顧技巧，另外，由於無法說話引發角色功能及身體心像改變問題，筆者依據個案的需求，提供正向的情緒因應策略，有效增進個案自我調適，協助度過無望感的危機，重建個案的希望、增進生活上品質。同時藉此提供護理人員日後照護此類個案之參考。

This study focused on the adaptation process of a patient who got brain tumor complicated with aphasia druing the treatment at the hospital. Roy Adaptation Model was applied to the care of 23-year-old female patient from May 17 to June 16 in 2004 at a ward of neuro-medicine. Data were collected by physical assessment, conversation and observation to identify the problems of the altered body image and the failure of role-playing and hopelessness. Furthermore, the complete evaluation and nursing intervention were the main measures not only to help the patient's family members to comprehend her condition, but also to provide them the related information for nursing skills. In addition, we assist the patient to pass through the hopelessness crisis by means of providing her positive emotion management and self-adaptation in order to improve her life qualities and also take this case for the nursing staff's reference.
