A Project to Improve Oral Care in Oral Cancer Patients with Flap Reconstruction Post-Operation

摘要

近年來台灣地區口腔癌呈現逐漸增加趨勢，治療方式以手術為主，良好的口腔護理能使傷口復原。本專案目的為改善口腔護理以提升口腔癌病人口腔照護。現況中發現護理人員對口腔護理認知的正確率僅72.4%，護理人員執行口腔護理技術的正確性僅達67.6%，口腔癌病人對口腔護理之滿意度僅4.0分，舒適度亦僅4.0分，口腔黏膜完整性為23.62，顯示護理人員對於口腔癌病患手術後口腔護理的認知與技術仍有待提升。因此，綜合以上結果，進行本專案之改善方案：(1)安排護理人員口腔照護訓練，(2)製作口腔癌皮瓣移植術後口腔護理衛教單張，及(3)持續監控。經過本專案改善後護理人員對口腔護理認知由72.4%增加為96.5%，護理人員執行口腔護理正確性由67.6%增加為95.4%，口腔癌病人對口腔護理之滿意度由4.0分提升為4.5分，舒適度由4.0分提升為4.6分，口腔黏膜完整性為17.21。因此，透過此專案改善後，達到提升口腔癌病人手術後的口腔護理。

The incidence of oral cancer in Taiwan has gradually increased these years. Surgery is major treatment, and good oral care can promote wound healing. The purpose of the project was to improve oral care for oral cancer patient with flap reconstruction post-operation. Before this project, the accurate percentage of the nurses' knowledge on oral care was 72.4%, the accurate percentage of the nurses' skill on performing oral care was 67.6%, the patients' satisfaction score was 4.0 and comfort score was 4.0, and the score of the integrity of oral mucus membrane was 23.62. These data indicated that nurses' knowledge and skills of oral care needed to be improved. After analyzing the current procedures, the proposed improvement project included: (1) arranging nurses to receive the oral care training; (2) developing an instruction booklet on oral care for oral cancer patients with flap post-operation; (3) continuously auditing oral care. After the improvement project, (1) the accurate percentage of the nurses' knowledge on oral care has increased to 96.5%, (2) the accurate percentage of the nurses' skills of oral care has increased to 95.4%, and (3) the patients' satisfaction score has increased to 4.5, comfort score was 4.6, and (4) the score of the integrity of oral mucus membrane has increased to 17.21. The project has improved the nursing quality of oral care.
