摘要

化學治療藥物屬高危險性用藥，兼顧病人與工作人員之安全有其必要。本專案旨在建立一個安全的給藥環境，提昇化學治療給藥安全。針對某醫學中心口腔外科病房，以觀察法及問春調查法稽核護理人員執行化學治療給藥過程，確認主要問題為護理人員不知道有標準流程、單位未推行化學治療給藥護理標準、病房備藥空間受限、護理人員認為穿著防護裝備麻煩及護理人員過去經驗沒有一起核對也沒有發生錯誤。經實施在職教育、建立稽核制度及設置安全加藥環境等解決方案後，化學治療給藥安全達成率由30%提昇到92.5%，亦提昇護理人員對於化學治療的相關知識及技能，建立安全給藥環境。本專案可作為其他非腫瘤專科病房推廣化學治療給藥安全作業標準之參考。

Chemotherapy is one kind of high-risk medications. It is necessary to consider the patients and care providers' safety during medication. The purpose of this project is to establish a safe environment for medication and promote chemotherapy medication safety. Observation and questionnaire were used to audit nurses while administering chemotherapy at an oral surgery unit of a medical center. The following problems were identified: nurses' lack of knowledge of standard procedures, lack of standard protocols for chemotherapy in the unit, limited space for preparing medications in the unit, nurses' feeling troubleome for wearing protective gowns and past experiences of no medication errors without double checking orders. Through performing continuing education, establishing the audit system, and safe environment for medication, the safety medication rate for administering chemotherapy increased from 30% to 92.5%. In addition, the nurses' knowledge and skills of chemotherapy were also improved. The project of chemotherapy medication safety can offer strategies and references to care providers who administer chemotherapy in other non-oncology units.
