癌症病人往往忍受著癌痛的折磨,PCA裝置是癌痛控制的重要工具,並能迅速緩解癌痛。專案旨在提升護理人員執行PCA裝置的完整性。經現況分析發現護理人員執行PCA裝置流程的完整性僅69.9%,原因為使用的規範不符臨床使用、缺乏PCA裝置相關教育訓練及缺乏PCA合適的輔助工具。本組於2015年6月1日至10月30日訂定及執行其作業流程、舉辦PCA之醫學討論會及實務訓練、製作新作業核對表及記錄表,並進行各表單的彙整作業後,執行PCA裝置的完整性從69.9%提升為99.3%,而認知調查也由74.0%提升96.4%,達本專案設定目標,並將此方案推廣至執行此裝置的病房,提供病人無痛的優質照護。
Cancer patients experience cancer pain which causes physical and mental suffering. Patient Controlled Analgesia (PCA) is an important method for cancer pain control and rapid cancer pain relief for cancer patients. However, the completion rate of PCA implementation protocol among oncology nursing staffs was 69.9%. Several issues were noted regarding the incomplete PCA implementation protocol such as inadequate guidelines and lack of relevant education, training, and PCA implementation aiding tools. The purpose of this project is to improve the completion rate in implementing the PCA pain management method among the oncology nursing staffs. The project started from June 1, 2015 to October 30, 2016. After assessing the then-existing protocol, the research group developed four tasks which are as follows: (1) establish an appropriate PCA implementation procedure standard; (2) organize the PCA training ad seminars; (3) build the implementation procedure checklist and logging system; and (4) systemize all PCA related forms. Substantial improvements were observed afterwards which are as follows: (1) the completion rate of PCA implementation protocol among oncology nursing staffs increased from 69.9% to 99.3%; and (2) the knowledge of PCA methods among oncology nursing staffs increased to 96.4%. The project reached its expected goals and we hope to see betterment in this type of pain management on cancer ward in the future.
