晚期子宮頸癌婦女接受同步化學放射線治療期間的生活經驗

The Lived Experience of the Women with Advanced Cervical Cancer during Concurrent Chemoradiotherapy

摘要


本研究目的為探討晚期子宮頸癌婦女接受同步化學放射線治療期間之生活經驗。採質性研究設計,於台北市某一醫學中心進行,以立意取樣選取七位初次診斷晚期子宮頸癌,並即將接受同步化學放射線治療的婦女。研究者為此醫學中心婦科病房護理師,自個案住院接受期別檢查至治療結束後第一次回診約八至十週間,以主護護理師的身分,在照護過程的自然情境中,運用多次非結構式訪談及實際觀察的方式收集資料,完成每位研究對象12至14篇之會談過程實錄。資料分析採現象學研究法搭配內容分析法進行。晚期子宮頸癌婦女接受同步化學放射線治療期間的生活經驗包括:面對確立診斷的複雜情緒、惶恐眼前即將進行一場硬仗、陷入第一段展開的苦戰、跨入第二段最嚴厲的苦戰、及欣喜打完一場艱困硬戰卻心悸猶存。晚期子宮頸癌婦女面對癌症的診斷,經歷艱苦的治療過程,均為身、心、社會的高度挑戰,在治療結束後,仍害怕復發。本研究建議健康照顧者理解其憂慮與困難,以協助病患順利如期完成治療。此外,本研究建議在治療結束後,應針對害怕復發進行措施處置,以增進後續存活期間的生活品質。

This study aims to investigate the life experience of the women during their concurrent radiochemotherapy for advanced cervical cancer. This study adopted the qualitative design and purposive sampling in a gynecology ward of a medical center in northern Taiwan. Seven women who were initially diagnosed with advanced cervical cancer and were expecting concurrent radiochemical radiotherapy were recruited. The first author is a nurse practitioner in the research field, who was the primary nurse and provided nursing care in a normal setting during their treatment. Unstructured interviews and observations were used to collect data for content analysis with the phenomenology perspective. Twelve to fourteen interview notes were produced from each patient. The analysis revealed five main aspects of their life during concurrent radiochemotherapy: (1) complicated emotions towards the cancer diagnosis, (2) strong fear of facing the coming battles, (3) suffering from the first battle, (4) embarking on the worst struggling stage of the tough second battle, and (5) mixed feeling of triumph and fright after each battle. All participants experienced very tough challenges in the physical, psychological and social aspect. After the treatment, there is still the fear of cancer recurrence. Health professionals should understand patients' worries and difficulties in order to assist patients to complete the treatment. We also recommend interventions regarding fear of recurrence to secure their quality of life as cancer survivors.
