核醫乳房掃描檢查應用女性乳癌患有乳癌復發之應用--病例報告

摘要


一位52歲女性乳癌患者,左側乳房因乳癌做外科手術切除,外科手術後接受化學治療及放射治療,之後,曾做過乳房超音波和乳房X光攝影,發現左側乳房有不明組織硬塊,之後安排99mTc-MIBI乳房掃描(92年4月),以評估是否有乳癌復發現象;於患者靜脈注射20mCi 99mTc-MIBI,等待10分鐘後用gamma camera 分別針對患者胸部取得左右和前側的影像,之後並加做SPECT及3D處理,發現左側乳房胸壁表淺部有99mTc-MIBI活性聚積,疑似乳癌復發,之後安排電腦斷層(92年5月),電腦斷層報告發現在病患手術後在發現左側乳房有輪廓不明軟組織硬塊,病理報告證實為乳癌。

In the early 1990s Sestamibi became commercially available, Several articles have been published on the both planar SPECT imaging in the detection of breast cancer. SPECT imaging may be valuable when planar images are not conclusive(especially when the lesion is in projection of the heart or upper part of the live on planar view) to better characterize or multifocal lesions or in the detection of axillary metastases. Radiologic mammography is a well-established diagnostic produce that is relatively inexpensive, widely available, and serves aw the best screening tool available today for breast detection, cause the limited of radiologic mammography(such as patient with iatrogenic architectural distorsion of the breat..), scitimammography(99mTc Sestamibi scan) should rather be considered as a complementary diagnostic procedure to breast cancer.
