使用SmartPrep執行電腦斷層冠狀動脈血管攝影及與心導管的比較

Comparative Study between Catheterization Angiography and Coronary Computed Tomographic Angiography Using SmartPrep Technique

摘要


本研究的目的是要瞭解使用SmartPrep的技術,實行電腦斷層冠狀動脈血管攝影(Cardiac CTA;CCTA)的成效,並將其結果與心導管攝影比較。針對94年7月至95年8月期間,共13位患者,使用64排電腦斷層掃瞄儀實行CCTA,同時在一個月內做過心導管,並收集兩項檢查的影像,請放射科醫師及心臟科醫師,使用美國心臟協會(AHA)的15-Segment模式爲基礎,以Segment-by-Segment且不記名的方式分析比較。CCTA的切面厚度(slice thickness)爲0.625㎜,管球旋轉速度(Rotation Time) 0.35秒,以每秒5㏄的速度注射顯影劑,掃瞄時間約5~7秒,結果發現有86%的影像是吻合,而影像無法吻合的原因爲嚴重鈣化、置入支架、冠狀動脈出口位置異常及冠狀動脈完全阻塞所造成。

The purpose of this study is to compare the results of coronary catheterization angiogram and cardiac computed tomographic angiography (CCTA)using the SmartPrep technique. From July 2005 to August 2006, total of 13 patients were included in this study using a 64 multi-detector CT scanner. Interpretation of the CCTA and invasive coronary angiograms was based on the American Heart Association 15-segment model and were assessed by our radiologist and cardiologist respectively, The CCTA protocols was as below: slice thickness 0.625 mm, rotation time 0.35 seconds, injection rate 5 cc per second, scanning time 5-7 seconds, Our results showed at 86 % of the segments were compatible in both CCTA and invasive coronary angiographic findings. The causes of mismatched segment were heavy atherosclerotic calcifications, npresence of coronary artery stents, congenital coronary artery anomalies and total coronary artery occlusion.
