利用再一次的排尿膀胱尿道攝影來診斷隱藏性膀胱輸尿管之逆流

摘要

利用再一次的排尿膀胱尿道攝影探討是否提高隱藏性膀胱輸尿管逆流的診斷率，以及是否提高膀胱輸尿管逆流的級數之診斷正確率。研究中共收集了200名患者。其中男性有112名，女性有88名。年齡從一星期到14歲，平均年齡為2歲。檢查時讓顯影劑經由滴輸液導管和尿管注入膀胱裡，利用透視觀察當患者開始解尿時就拍攝影像。解完尿後重新將顯影劑注入膀胱，再次利用透視觀察患者於開始解尿時有無膀胱輸尿管逆流。在200名患者中，結果發現有膀胱輸尿管逆流的患者總共40名，總共50條輸尿管，其中男性有20名，女性有20名，第一次灌注顯影劑排尿時就發現有膀胱輸尿管逆流的患者總共26名，有35條輸尿管，第二次灌注顯影劑排尿時才發現有膀胱輸尿管逆流的患者總共14名，有15條輸尿管。檢查時間總平均為17.5分鐘。再一次的排尿膀胱尿道攝影能夠提高提高隱藏性膀胱輸尿管逆流的診斷率，以及提高膀胱輸尿管逆流的級數之診斷正確率，而且檢查所需的時間平均為17.5分鐘，並不需要花費太多時間來做檢查。

By using cyclic voiding cystourethrography to evaluate if it can be more efficient in diagnosing occult vesicoureteric reflux, and if it may increase the validity in the staging of vesicoureteric reflux.
There are 200 patients in this study, 122 boys and 88 girls, aged 1 week to 14 years, 2 years on the average. Initial VCUG was performed by contrast medium instillated into the urinary bladder via foley catheter connected to an IV set. Voiding through the catheter with fluoroscopy and radiographic images were taken. The procedure was repeated and any vesicoureteric reflux was observed by fluoroscopy.
Of the 200 patients studied there are 40 patients with vesicoureteric reflux with a total of 50 ureters refluxing, 20 boys and 20 girls. There are 26 patients (total of 35 ureters) diagnosed to have vesicoureteric reflux during the first VCUG. There are 14 patients (total of 15 ureters) diagnosed to have vesicoureteric reflux during second VCUG. The average time of the study time was 17.5min.
Cyclic voiding cystourethrography can increase the efficiency of diagnosing vesicoureteric reflux and the validity of diagnosing the stages of vesicoureteric reflux as well.
