全方位的骨質疏鬆症評估-骨質密度檢測與立即脊椎評估技術之結合

One-stop Osteoporosis Evaluation-Combined Bone Mineral Density Measurement and Instant Vertebral Assessment

摘要


脊椎骨折幾乎是骨質疏鬆症最常見的結果,也是伴隨再發生骨折一個重要的危險因子。如果病人骨質密度的減少和脊椎骨骨折,意味著未來骨折的危險因子將會增加,由此可知對於脊椎骨折的評估與骨質密度的評估兩者相輔相乘使用的重要性。然而大部分的脊椎骨折是在無聲無息下進行的,而且一般的骨質密度檢查並不是常規地包含脊椎側面像,立即脊椎評估(IVA; Instant Vertebral Assessment)技術,是一種應用於雙能量X光吸收測量儀(DXA; Dual X-ray absorptiometry)的新技術,這個技術提供了一個快速地、低劑量、準確的脊椎骨折的評估。臨床上的資料證實,使用雙能量X光吸收測量儀所獲得的側面脊椎的影像(lateral spine image)是可行的,此新方法的影響在臨床病人身上的評估是可以討論的,結合骨質密度測量的評估與IVA技術應用於脊椎骨折的評估,使的我們可以更準確的診斷與評估病人,是否有骨質疏鬆症的疾病及脊椎骨折的風險,以利於早期預防和治療。

Vertebral fracture is common finding of osteoporosis, and is another important risk factor for subsequent fractures .When a patient has decreased bone mineral density and vertebral fracture, it implies an increased risk of future fractures. From these findings we can deduce the dual importance of bone mineral density and vertebral fracture assessment. Most vertebral fractures are asymptomatic, conventional bone mineral density assessment does not include lateral spine X-rays projection. Instant Vertebral Assessment (IVA), a new technique using DXA (Dual X-ray absorptiometry), provides a rapid, low-radiation dose, and accurate assessment of vertebral fracture. Clinical data supporting the feasibility of visual evaluation of lateral spine images obtained using a fan-beam DXA system are also reviewed, and the potential positive impact of this new methodology on clinical patient evaluation is discussed. Combining the measurements from bone mineral density measurements and IVA in the vertebra, we are able to diagnose and assess accurately the risk of osteoporosis and vertebral fractures, Favors the early prevention and the treatment.
