應用田口方法評量Spider View造影之最佳調整參數

Optimization of Conroy Angiography for Spider View via Taguchi Methodology

摘要


冠狀動脈疾病是指輸送血液及氧氣至心臟肌肉的血管發生狹窄或阻塞導致心臟肌肉缺氧之現象。而現今快速又有效的診斷方法中是以心導管冠狀動脈攝影,可直接評估阻塞的程度及部位,而冠狀動脈攝影術中的Spider View是診斷左主冠狀動脈、左前降支開口與左迴旋支開口最佳的影像,但常因為某些調控因子的改變而無法有效的將其分開,造成無法正確診斷出病灶。因此,本文是應用田口方法,用模型假體模擬冠狀動脈攝影術中Spider View的造影,依照田口方法歸納為18組並定出六項控制因子進行評量Spider View造影中對左主冠狀動脈、左前降支開口與左迴旋支開口病灶能有更加清晰之影像呈現的最佳調整參數,並做確認驗證以驗證實驗結果之再現性。結果顯示,原設定的S/N比值為21.2dB、累積誤差為1.06%。而最佳化設定後之S/N比值為24.5dB、累積誤差為0.94%。故根據本文之結果,最佳影像品質水準組合為不使用濾片、左側位攝影角度(LAO)70°、後側位攝影角度(CAU)30°、尖峰電壓110kVp、電流與時間1.5mAs、管球與影像倍增管之距離108cm;因此,得知最佳參數組合優於原設定參數,故將此應用於臨床冠狀動脈攝影術中,可以有效提昇介入性診療時Spider View影像之品質,以造福更多的冠狀動脈疾病病患。

Coronary artery disease means that blood vessels, which transport blood and oxygen to muscles of the heart, are narrow or clotting. Currently, one of the fast and effective diagnosis methods is Cardiac Catheter Coronary Artery Angiography, which can directly evaluate the region and extent to clotting of blood vessels. The best view in clinic for diagnosing left main trunk, LAD and LCX proximal sites is spider view, which is one view of coronary angiography. Changes of certain factors, however, give rise to diagnose ostium lesion of these three vessels not accurately. Therefore, the research was to use model to imitate spider view of coronary angiography, then, on the basis of Taguchi Methodology, generalizing for eighteen groups (L18) and six control factors with an eye to measuring the best adjustment parameters of spider view for the ostium lesions of three vessels shown more clearly. Test and verify the reproductivity of the result under verification then. The result showed that original setting number, S/N ratio, was 21.2 dB, cumulative error was 1.06%; and, the S/N ratio was 24.5 dB, cumulative error was 0.94% both of which were done after optimizing. In accordance with the result of the research, the best combining factors for the best image quality were without filter, the angle of photography for left side: LAO 70°, the angle of photography for rear side: CAU 30°, the peak voltage: 110 kVp, time and electric current: 1.5 mAs, the distance between tube and image detector: 108 cm. Thus, it is the best combining parameters were better than the original ones. Therefore, we took this applying to Cardiac Catheter Coronary Artery Angiography, then, elevating the quality of image of spider view, making a preferable treatment for patients would be expected.
