多排偵測器電腦斷層對耳咽管開放症之應用

The Application of Multi-detectors CT in Patulous Eustachian Tube

摘要


耳咽管是呈封閉的狀態,只有在吞嚥、打哈欠時因為肌肉的收縮而打開,這時中耳腔就能藉機與外界做壓力的平衡。而耳咽管開放症是因為中耳腔持續與鼻咽相交通,而病人呼吸時會覺得「聽得到自己的呼吸聲」。根據國外的研究報告發現,要準確的診斷此疾病,需藉由坐立式的電腦斷層,但由於台灣的電腦斷層幾乎是平躺式的檢查台,故要順利的檢查出此病症並不容易。本研究要藉由多排偵測器電腦斷層影像多平面重組功能與改變病患受檢姿勢,而將耳咽管開放症診斷出來。本研究受檢病患採立姿檢查,並將病患頭顱固定於頭架內。掃描後將原始影像傳入影像處理工作站,再利用多平面重組功能,沿著咽孔到鼓室孔連線,將其開放的耳咽管顯示出來。本研究為期一年,共收集9位有耳咽管開放症狀而做鼻竇電腦斷層的病患,其中男性有4位,女性有5位。雖然電腦斷層檢查台為固定的,但我們可以改變病患的受檢姿勢,並利用電腦斷層影像重組功能,來達到診斷耳咽管開放症的效果。

The Eustachian tube is normal closed, and open when people swallowing or yawning because of muscle contraction. When the Eustachian tube is open, the pressure of Middle ear cavity will balance with the pressure outside of the body. Patulous Eustachian tube is due to the middle ear cavity, always connect with nasopharyngeal and the patient will feel can hear the breathing of himself. According to some overseas reports is by CT scan when patient with sitting posture. However, CT in Taiwan almost with supine table, so it is not easy to diagnose the disease with general CT scans. This research will focus on the patient position modification when scan with multi-detectors CT, and combine to use multiplanar reconstructions (MPR). We hope can find out a good method to diagnose patulous Eustachian tube disease accurately. Research the patient scan with standing position and fix the brain of the patient in the head frame. We send the CT original date to workstation, and use MPR function along form the pharyngeal orifice to the tympanic orifice connection, showing the patulous ET. The study of sinus CT scans identified over the 1-year period. Form this group, we use CT scan to determine 9 patients are suffering from patulous ET including 4 males and 5 females. Although the CT examination table is fixed,we still can change the position of the patients and using CT MPR to achieve the effect of disease diagnosis for patulous ET.
