比較鉛防護對於站立式胸部X光攝影輻射有效劑量的影響

Comparisons of Effective Radiation Dose with/without Lead Shield in Patients with Standing Chest X-ray Examination

摘要


本研究的目的為評估站立式胸部X光攝影時,使用腹部鉛屏蔽前、後,受檢者所接受之散射輻射劑量評估。將54顆TLD黏貼於腹部鉛屏蔽上,分別於輻射防護屏蔽內(影像偵檢器端、屏蔽)、外(X光管球端、未屏蔽)側,上、中、下,左、中、右,各貼上3顆TLD。收集統計樣本由2015年02月05日至2015年02月11日,總共收集534人次,以臨床攝影條件進行攝影,記錄攝影部位、攝影條件kVp及mAs,實驗結束後用HARSHAW 3500 TLD Reader 進行劑量計讀。結果顯示,腹部鉛屏蔽外側之9 點之累積劑量範圍是0.4~6.4 mSv,腹部鉛屏蔽內側9 點之累積劑量範圍是0.19~0.73 mSv,經換算,腹部鉛屏蔽外側之上與中層位置(分別約為腹部與性腺位置),平均每人次約分別接受11.99 μSv 與3.22 μSv等價劑量;腹部鉛屏蔽內側之上與中層位置,平均每人次約分別接受1.37 μSv 與0.62 μSv等價劑量。由本研究結果可知,站立式胸腔X 光攝影,使用腹部鉛屏蔽後,可以有效防護腹部所接受之散射輻射劑量,明顯地降低受檢者所接受之有效劑量。

The purpose of this study was to evaluate the scatter radiation dose received by patients with lead shield undergoing the chest PA X-ray examination. A total of 54 TLDs are adhered to the lead shield; 27 TLDs are attached in both inner and outer sides of the shield. The data collection was performed from Feb 5, 2015 to Feb 11, 2015, and image parameters (type of examination, kVp and mAs) of 534 patients were collected. After measurement, all TLDs were read using a HARSHAW 3500 TLD Reader. The result shows that the dose ranges of the outer and inner side of the lead shield were 0.4~6.4 and 0.19~0.73 mSv, respectively. For the outer side of lead shield, the measured doses of abdomen and gonad positions were 11.99 and 3.22 μSv, respectively. For the inner side of lead shield, the measured doses of abdomen and gonad positions were 1.37 and 0.62 μSv, respectively. In conclusion, the measured doses of abdomen and gonad positions were significantly reduced after the lead shield applied.
