術中使用移動式電腦斷層掃描儀執行腦部血管攝影評估動脈瘤夾除術後之成效-案例報告

Performing Brain CTA by Portable CT to Evaluate the Efficacy after Surgical Clipping in the Operating Room - Case Report

摘要


腦部電腦斷層血管攝影術診斷腦部血管疾病是目前最快速、準確、非侵襲性的方式,以往在手術室中礙於設備以及軟體的限制而無法立即執行此項檢查,目前臺灣引進了第一台全身型移動式斷層掃描儀,能夠快速地完成全腦動脈的掃描以及影像後處理,進而提供更完整的影像資訊以確認手術成效。本案例展示一位49 歲女性病患因為急性中腦動脈血管瘤破裂後於動脈瘤夾除手術中執行腦部電腦斷層血管攝影術的案例,目的在於呈現此全新的應用方式以供臨床參考,此做法不僅可以讓醫師立即評估手術成效,亦可避免因為檢查而造成移動以及感染的風險。

Using brain computed tomographic angiography (brain CTA) for diagnosis of brain vascular disease is a rapid, accurate, non-invasive way. In the past, brain CTA cannot be performed in the operating room because of the lack of suitable equipment. Now, because of the development of whole-body portable CT, whole brain artery scan and image post-processing during operation could be quickly achieved. Three-dimensional volume rendering (3D-VR) and multi-planar reconstruction (MPR) provide more integrated information for evaluation of surgical outcome. In this study, a case of 49-year-old female patient who had ruptured aneurysm in middle cerebral artery underwent brain CTA to check the blood flow after surgical clipping successfully. This application of portable CT provides feasibility to allow surgeons to immediately assess the result of the operation, and avoid critically ill patient transport and risk of infection.
