摘要


近年來隨著乳房攝影技術之進步而發展出顯影增強能譜乳房攝影(contrast-enhanced spectral mammography; CESM),此為一種注射顯影劑的乳房攝影檢查,將顯影劑注射到受檢者體內以提高隨乳房腫瘤發展而新生成的新生血管可見度,再以二種不同能量射束曝露後,經過贅減影像處理而得到雙能譜贅減影像。此技術結合一般乳房攝影對鈣化分析的獨特性和檢查的方便性,與在核磁共振(magnetic resonance imaging)注射顯影劑後針對腫瘤的顯影得到精準的影像分析特性,綜合兩項檢查的特性得到具有準確的診斷與研究價值。本案例為一45 歲女性在健康檢查的超音波影像發現疑似乳房惡性變化,經過門診安排CESM、核磁共振和病理檢驗報告得到相同的診斷結果。由此例介紹CESM可提高對乳房腫瘤的鑑別診斷。

Recent advances in breast imaging technique evolved newer diagnostic modality of the breast, including the contrast-enhanced spectral mammography (CESM). CESM combines the benefit of both contrast medium administration and conventional mammography by obtaining a subtraction image from the dual-energy exposure within a single study, which allowed better identification of contrast medium enhancement from breast tumors owing to angiogenesis. The combination of both the unique ability of conventional mammography in identifying suspicious calcifications and also the ability for contrast enhancement in identifying tumor mass; both enhanced the clinical diagnostic and research capability of CESM. In this case an ultrasound image of a 45-year-old woman was found suspected breast malignancy. Later clinic arranged CESM, magnetic resonance imaging, and pathological test reported the same result. We introduced this CESM example to improve differential diagnosis of breast tumor.
