A 56-year-old male had history of type 2 diabetes and chronic kidney disease. The patient was brought to our emergency department because of progressive dyspnea. Fever, leukocytosis, and elevated C-reactive protein were noted. Besides, neck mass with swelling and pain was noted and the condition persisted. Gallium-67 (Ga-67) scintigraphy was performed to the patient for fever of unknown origin. The Ga-67 scintigraphy revealed extensive hot gallium uptake in the anterior and left neck extending into upper mediastinum and mild uptake in the mediastinum and surrounding the heart. Diffuse moderate to hot uptake in both kidneys and hot uptake rigth lower abdomen is also noted. The presented case demonstrated an incidental finding of Ga-67 uptake surrounding the heart (pericarditis) on Ga-67 scintigraphy.
