探討乳癌病患於錐狀射束電腦斷層旋轉軸與平移軸擺位誤差轉換之可行性

To Explore the Feasibility of the Position Deviation Conversion between Rotation Axis and Translational Axis in Cone-Beam Computerized Tomography for Breast Cancer Patients

摘要


本研究的目的是利用一個旋轉軸與平移軸合併轉換為平移軸的公式,套用在乳癌病患之錐狀射束電腦斷層(cone beam computed tomography,CBCT)的數值,比較轉換前後數據之差異去評估該公式能否直接應用在本單位,接受乳癌放射治療之病患於沒有旋轉軸修正功能的治療床上。121位接受乳房放射治療的病患,共653組CBCT數值,套入Sarkar B等人於2018年3月發表的公式,使用學生t檢定比較轉換前後之數據差異;轉換前的數據已是臨床應用於病患,若p值呈現顯著,那麼判定該公式無法直接使用。根據轉換公式得到三維各軸轉換前後的平均數與p值的結果如下:-0.54vs1.53,p<0.001、1.56vs-0.15,p<0.001,-1.07vs0.06,p<0.001,單位為mm。結果顯示:本單位無法立即在臨床乳癌病患的CBCT數據使用此公式,於無法執行旋轉軸修正之治療床上。

The aim of this study is using a formula of converting the rotation axis and translation axis into the translation axis at the values of cone-beam computed tomography (CBCT) for the breast cancer patient, comparing the difference between the data before and after the conversion, exploring whether the formula can be directly applied for the patient who receives breast cancer radiotherapy (RT) on the treatment couch without the rotation axis correction function. There were 653 CBCT data of 121 patients receiving breast RT, using the formula that was published by Sarkar B(March 2018) et al. The data difference before and after the conversion is evaluated by the Student's t-test. The before data were using on the patients in clinical, then the formula could not use when the p value was significant. The mean and p values before and after the three-dimensional transformation according to the conversion formula as follows: -0.54 vs 1.53, p <0.001, 1.56 vs -0.15, p <0.001, -1.07 vs 0.06, p <0.001, mm, respectively. The results were shown that this department cannot apply the formula at CBCT data for breast cancer patients, where the treatment couch cannot perform rotation axis correction, immediately.
