利用心臟磁振造影評估心包脂肪體積的可行性:豬心研究

The Feasibility of Magnetic Resonance Imaging Sequence on Measuring Pericardial Fat Volume: Pig's Heart Research

摘要


過去的研究已經證實心包脂肪(Pericardial fat, PF)與心肌的三酸甘油脂(triglyceride)含量有關,亦與冠狀動脈疾病有直接的關係。目前臨床上可利用心臟超音波(Ultrasonic)、電腦斷層(Computed tomography, CT)及心臟磁振造影檢查(cardiovascular magnetic resonance imaging, CMR)的影像來定量PF的含量。本研究主要利用9顆體外豬心,分別進行CT及三組不同序列的CMR進行掃描,將所獲得之影像利用半自動化分析軟體進行PF體積定量分析。結果顯示CT影像所計算出的PF與三組不同CMR序列影像所計算的PF並無統計上的差異,其分別為Cine序列(45.03±29.55 vs. 30.69±30.70 gm, p = 0.222),單次激發快速自旋回波序列(45.03±29.55 vs.34.15±29.17 gm, p =0.546)及快速自旋回波序列(45.03±29.55 vs. 40.42±36.91gm, p = 0.546)。本研究成功藉由動物模型比較CT與CMR影像兩者在分析PF的差異,發現CMR的檢查所計算的PF含量與CT所測量的結果並無統計上的意義,代表不具游離輻射的CMR檢查可用來定期追蹤檢查病患的PF改變。

Previous studies have demonstrated that the Pericardial Fat (PF) is not only related to the triglyceride content of myocardium but also has a direct correlation with various cardiovascular disease. Several imaging modalities, such as cardiac ultrasound, computed tomography (CT) and cardiovascular magnetic resonance (CMR), are feasible to quantify the PF. The aim of this study is comparing PF measurements from CT and CMR with three routine pulse sequences in 9 ex-vivo pig hearts. Semi-quantitative analysis was performed to quantify the PF from CT and CMR images. There were no significant differences in PF from CT and Cine sequence (45.03±29.55 vs. 30.69±30.70 gm, p = 0.222), single-shot fast spin-echo sequence (45.03±29.55 vs. 34.15±29.17 gm, p=0.546) and fast-spin echo sequence (45.03±29.55 vs. 40.42±36.91gm, p = 0.546), respectively. Our results showed that structural measurement sequence (single-shot fast spine echo or fast-spin echo sequence) and functional imaging sequence (cine) are both feasible to assess the PF comparing in CT. The free of ionizing radiation of CMR is particular benefit to patients who are needed to assess PF in long-term follow-up.
