Arterial Pseudoaneurysm is a rare vascular complication after pancreas transplantation. A ruptured pseudoaneurysm will result in serious bleeding. Delayed diagnosis and treatment in the rupture of arterial pseudoaneurysm might lead to life threatening condition. A 33-year-old female received pancreas transplantation 11 years ago. She suffered from large lower gastrointestinal bleeding, bloody/tarry stool and unstable vital signs. Angiography demonstrated rupture of the Y graft pseudoaneurysm. Initially, the pseudoaneurysm of the Y graft vessel was embolized with coils successfully. Unfortunately, the recurrent bleeding of the Y graft pseudoaneurysm occurred. Embolization with coils and covered stent was used to stop bleeding successfully. No recurrent bleeding was found at 3 months follow-up.
