摘要

本文之案例為急診照會放射診斷科診斷急性腦中風（acute ischemic stroke, AIS）患者時，因主動脈剝離（aortic dissection, AD）造成次發性腦中風（ischemic stroke secondary, ISS）。診斷AIS採取特定之電腦斷層掃描程序（scan protocol），提供一系列影像學診斷。AD導致AIS為罕見原因，若未即時發現而給予靜脈血栓溶解劑（rt-PA）或介入性治療是有可能致命的。為避免造成更嚴重之後果與後遺症，電腦斷層掃描檢查需診斷排除AD之可能性。

The case of the article is the diagnosis in patients with acute ischemic stroke (AIS) causing ischemic stroke secondary subsequently due to aortic dissection (AD). The diagnosis in AIS adopts specific computed tomography scan protocol to provide a series of imaging.AD leading to AIS is considered as a rare cause. It can be fatal if intravenous recombinant tissue plasminogen activator (rt-PA) or interventional therapy is not given immediately. In order to avoid severe outcome and sequels, computed tomography needs to be done to exclude the possibility of AD.
