Purpose. Because patients with difficult defecation have overlapping symptoms, selecting appropriate therapy on the basis of clinical history and findings from routine examination is challenging. The purpose of this study was to use defecography to evaluate anatomic abnormalities in patients with difficult evacuation.Methods. Between May and December 2004, we enrolled 57 patients with difficult defecation. These patients underwent radiography at rest, during squeezing and straining and after defecation. The entire process of evacuation was assessed with fluoroscopy. Anatomic abnormalities were recorded.Results. There were 10 men with a mean age of 64.3 years (range, 44-85 years) and 47 women with a mean age of 49.9 years (range, 20-79 years). We found 37 anterior rectoceles, 10 posterior rectocele 28 sigmoidoceles, 18 mucosal prolapses, 14 intussusceptions, 9 rectal prolapses, 14 paradoxical puborectalis contractions, and 10 enteroceles.Conclusions. Defecography was a low invasive, safe, and simple procedure that enabled dynamic evaluation of anorectum and surrounding structures during defecation. It was an objective method for examining patients with complex subjective symptoms and allowed us to know the reason which might cause difficult evacuation and could aid in selecting optimal therapy.
