Hypersensitivity Reactions to Oxaliplatin: A Prospectively Collected Study of 25 Cases Treated in One Institute

Purpose. As therapeutic results of oxaliplatin in the treatment of colorectal cancer are widely accepted, the allergic reactions to oxaliplatin are encountered more and more. This study aims at investigating whether or not the oxaliplatin should be discontinued when the allergic reaction occurs.Methods. We prospectively collected 25 cases of allergy to oxaliplatin that occurred between January 2006 and December 2007. We recorded and analyzed their chemotherapy courses, allergic reactions, and managements they received.Results. Eleven females and 14 males had hypersensitivity reactions. Twenty two were cases with stage IV colorectal cancer. All 25 cases had received modified FOLFOX 4 regimen. They encountered allergic reactions ranging from grade 1 to 3 of severity. Thirteen cases had grade 1, eight cases grade 2, and four cases grade 3 allergy. The managements to the patients who developed allergy were symptom oriented. The oxaliplatin infusion was withheld and substituted with fluid support. Steroids, antihistamines, and oxygen were administered to alleviate the allergic reactions. There was no mortality, and no one required intensive care. At the first occurrence of allergy, we had 88% success rate of completing the cycle. At the second occurrence of allergy, the success rate of completing the cycle was 60%. Overall, there was 76% success rate in sustaining the oxaliplatin treatment program despite the development of hypersensitivity reactions.Conclusions. The allergic reactions to oxaliplatin can be appropriately managed without any sequelae. Prolongation of the infusion time of oxaliplatin is an effective measure to manage hypersensitivity reactions. Overall, with appropriate managements, there was 76% (19 of 25 cases) could finish the entire oxaliplatin chemotherapy program even the adverse reactions developed.
