協助一位腎病患者選擇透析模式之護理經驗

Nursing Experience in Helping a Patient with End Stage Renal Disease Select the Mode of Dialysis

摘要


本文呈現一位因糖尿病併發末期腎病變患者,選擇適合自己的腹膜透析治療之護理經驗。筆者於98年8月3日至8月26日照護期間,藉由會談、觀察、身體評估及病歷查閱等方式收集個案資料,運用Gordon十一項功能性健康型態評估歸納出個案有體液容積過量、潛在危險性感染與抉擇衝突等健康問題。筆者運用主動關懷和同理心,建立良好護病關係取得其信任感,透過水份控制相關指導及適當透析,改善體液容積過量情形,提供完整透析治療模式資訊,以利個案選擇最適合本身之腹膜透析治療,並完成自我照護腹膜透析導管出口及更換透析液等訓練,以預防感染,將個案身、心與社會各層面之衝擊降至最低,使其終能返回家中自我執行治療,提昇其生活品質。

The article presents nursing experience with a patient with end stage renal disease due to diabetes mellitus who choose peritoneal dialysis as suitable renal replacement therapy. The author cared for the patient from August 3 to August 26, 2009, and information on the case were collected through interview, observation, physical assessment, and chart review, etc. Using Gordon's eleven functional health patterns revealed health problems including excessive fluid volume, risk of infection and conflict of choices. Through proactive care and empathy, the author established a positive nurse-patient relationship to obtain the patient's trust. By providing guidance about fluid control and proper dialysis, an improvement in fluid overload obtained. The patient was also provided with complete information on dialysis treatment modalities to facilitate the selection of the most suitable type of treatment. In this case peritoneal dialysis turned out to be the best option. The patient was also trained on complete self-care of peritoneal dialysis catheter and replacement of dialysis fluid to prevent infection, thus minimizing the physical, psychological and social impact at all levels. The patient was eventually able to return home and practice self-treatment, improving quality of life.
