Effects of Acupressure on Fatigue, Sleep Quality, and Quality of Life in Hemodialysis Patients

摘要

本研究目的在探討穴位按壓對血液透析患者之疲憊感、睡眠品質與生活品質之成效，採類實驗設計法，收案對象取自台南地區某血液透析中心。符合收案條件者共65人。隨機分配至實驗組（35人）或對照組（30人）二組。實驗組接受一般血液透析常規照護及穴位按壓（湧泉穴、天柱穴、神門穴、內關穴），全程共15分鐘，而對照組僅接受一般血液透析之常規照護。共追蹤4週。研究工具包括個案基本資料、血液透析病患疲憊量表、匹茲堡睡眠品質量表、台灣簡明版生活品質量表。以SPSS（18.0版）進行統計分析包含百分比、卡方檢定、重複變異量分析等統計方法進行分析及檢定。本研究結果顯示於第四週追蹤時實驗組較對照組疲憊狀況（F=4.026, p=0.049）、睡眠品質（F=12.291, p=0.001）及生活品質（F=4.505, p=0.038）皆呈顯著改善。因此穴位按壓似乎可以有效改善血液透析患者之疲憊感、睡眠品質與生活品質。研究結果可提供護理實務工作者對血液透析患者照顧之參考。

This study investigates the effects of acupressure on fatigue, sleep quality, and quality of life in hemodialysis patients. This study used a quasi-experimental design with participants recruited from hemodialysis centers in the Tainan area of Taiwan. The patients were randomly divided into an experimental group (35) and a control group (30). The experimental group received standard hemodialysis treatment and 15 minutes of acupressure treatment (at the Yongquan, Tianzhu, Shenmen, and Neiguan pressure points), whereas the control group received only standard hemodialysis treatment. Patient follow-up was conducted for 4 weeks. The tools used in this study included individual case information, a hemodialysis patient fatigue index, the Pittsburgh Sleep Quality Index and the concise Taiwanese version of the Quality of Life Index. Statistical calculation methods, such as percentages, the chi-squared test, and repeated measures ANOVA were used and analyzed using SPSS18.0 software. The results demonstrated significant differences between the states of fatigue, sleep quality and quality of life among patients in the experimental group and those in the control group (F=4.026, p=0.049; F=12.291, p=0.001; F=4.505, p=0.038, respectively). The results indicate that acupressure improved fatigue, sleep quality and quality of life in hemodialysis patients. These results can be considered by nurse practitioners seeking alternative methods for the care of hemodialysis patients.
